Reflection 1: Glucose homeostasis & T1D

A friend of mine from home has Type 1 Diabetes, and learning about metabolic disorders and glucose homeostasis has deepened my understanding of what she experiences daily. I was previously aware that T1D is an autoimmune disorder in which the immune system attacks the pancreatic cells that produce insulin. Now, I understand why this is so detrimental to glucose regulation. Without insulin, glucose cannot efficiently enter cells, leading to elevated blood sugar and preventing glucose storage as glycogen. I now see why it’s so important for her to track carbohydrates and administer insulin after meals so that she can mimic normal insulin release and prevent hyperglycemia. Learning about glycogen synthesis and breakdown also clarified what happens when her blood sugar drops too low. Normally, when glucose levels fall, the pancreas releases glucagon, signaling the liver to break down glycogen into glucose. But due to her T1D, her body’s glucagon response is impaired so blood sugar can continue to decrease, increasing the risk of hypoglycemia. I also wonder if she has ever taken a glucose challenge test. I now know that her results would be a blood sugar level that remains high because insulin does not signal for glucose uptake and storage, whereas normally, the liver should take up blood sugar, store it as glycogen, and restore the blood sugar level.

    This is a very insightful and thoughtful reflection. It is powerful how learning more about our underlying physiology can make us more empathetic and understanding of what people with Type 1 Diabetes experience everyday. I think you have done a great job connecting the concepts of glucose regulation, insulin function, and glycogen metabolism to real-world challenges faced by your friend. Your explanation of why insulin is necessary for glucose uptake and storage after eating emphasizes how someone with Type 1 Diabetes has to constantly monitor their blood sugar levels and maintain a delicate balance. I also appreciate you mentioning the importance of glucagon and how it is critical in preventing hypoglycemia. This makes me appreciate the real-world application of some of the material we are learning in class.

    Your post really resonates with me, as I had a friend/teammate in middle school who also had type 1 diabetes. While I did not fully understand what it meant for him to have type 1 diabetes or why it was a big deal for him, looking back on it, I can better understand the concerns his parents had for him throughout the course of the day, especially given his role as an athlete. Because of his T1D, his body was not able to decrease his blood sugar after consumption of carbohydrates, so he had an insulin pump. However, he would also need to break down a lot of glycogen to perform at a high level, meaning that he needed to consume a good amount of carbohydrates. As you mentioned, what we learned in class gives me the ability to better understand what is going on biochemically in the body of a person with T1D and allows me to gain a better understanding of the complexities that come with living with T1D.
