Reflection 2: “Good” versus “Bad” Cholesterol

Before last week, most of what I knew about cholesterol came from public health messaging that framed it as something categorically harmful. I had heard that there were “good” and “bad” types, but I never actually knew what the difference was. Our discussion highlighted that what actually differs are the lipoprotein particles that transport cholesterol. LDL delivers cholesterol from the liver to tissues, while HDL returns excess cholesterol to the liver for excretion (reverse cholesterol transport per slide 10). Thus, high LDL is associated with increased risk of myocardial infarction and other adverse cardiovascular events, and high HDL is associated with a decreased risk of these events. Seeing these mechanisms mapped out helped me understand why the public-facing terms oversimplify the underlying biology. 

 

The unit also explained how some of our cholesterol comes from de novo synthesis in the liver, not just from diet. I particularly enjoyed learning how statins work as an inhibitor of HMG-CoA reductase because I know several people who take them to lower cholesterol. Overall, learning the biochemical basis of lipoproteins and the mechanism of statins helped refine my perspective from seeing cholesterol as simply “bad” to understanding it as a tightly regulated molecule. As such, these public health recommendations now feel less abstract, because I can connect them to the metabolic pathways that determine cardiovascular risk.

    I really like how you explained the difference between LDL and HDL. Prior to our lectures, I also knew little about the specific lipoproteins that transport cholesterol throughout the body. I also found de novo cholesterol synthesis intriguing, as I had only thought dietary intake played a major role.
